BAYADA Hospice is currently seeking a full time Hospice Community Liaison to work with our Concord, NH office. This position will cover the central NH area, to include but not limited to Sullivan County, Merrimack County, Hillsboro County

Liaisons at BAYADA interact with physicians, discharge planners, case managers, and clients and families at designated facilities to coordinate the initiation of hospice services.
