Overview

BAYADA Senior Living, a specialty program of BAYADA Home Health Care, is currently seeking an experienced Speech Language Pathologist, SLP, for a per diem opportunity performing home health visits in senior living communities in Forsyth County.  BAYADA Senior Living offers a comprehensive continuum of care to its clients, including traditional home care under the Medicare Part A benefit, as well as outpatient therapy services under the Medicare Part B benefit.  The foundation of the program is exceptional clinical care and relationship management as BAYADA clinicians become a true healthcare partner in the senior living communities.  Communities include assisted living, independent living, senior congregate living, 55+ communities, and intermediate care facilities for clients with intellectual disabilities. 

 

The BAYADA Senior Living setting supports team collaboration for care of our clients on a real-time face-to-face basis.  Unlike traditional community home care, BAYADA Senior Living team members do not drive to multiple homes in a day.  They may work in one to three settings in a day, and have the benefit of getting to know the residents and staff of their communities, and seeing their clients’ progress after discharge from service.

Responsibilities

  • Make home visits to clients in designated senior living communities.
  • Perform thorough evaluations to assess, diagnose, and provide therapeutic interventions for clients who are experiencing speech, language, communication, or swallowing difficulties.
  • Develop a treatment plan based on the evaluation, physician’s orders, and recommended goals.
  • Continually assess and revise the speech therapy care plan, and participate with BAYADA clinicians in the multidisciplinary care plan, as appropriate.
  • Educate and instruct patients, family members, or other patient representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
  • Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided, utilizing a state-of-the-art touch pad tablet.

Qualifications

  • A current North Carolina Speech Language Pathologist license.
  • Possess a certificate of clinical competence in speech from the American Speech and Hearing Association, or;
  • Have the equivalent educational requirements, as indicated by school transcript or diploma, and work experience necessary for such a certificate, and be awaiting the opportunity to take the American Speech-Language-Hearing Association examination for certification.
  • Interest in owning the wellbeing of assigned senior living communities.
  • Interest in and willingness to learn and deliver Medicare Part B therapy, as well as home health therapy.
  • Ability to work independently and manage time effectively.
  • Excellent interpersonal, communication, and relationship building skills.
  • Demonstrated ability to read, write, and effectively communicate in English.
  • Solid computer skills; prior experience with electronic medical records (EMR) preferred.
