Overview

BAYADA has an immediate opening for a Full-Time Clinical Transitional Care Manager working for Genesis accounts in VT. 

 

Highlights:

  • The Transitional Care Manager is responsible for identifying current and future clients and for facilitating the smooth transition of clients from facility to home.
  • As Transitional Care Manager, you will focus on building and maintaining relationships with clients and families, discharge planners, social workers and other facility personnel. 
  • Working with the staff of local BAYADA offices, you will lead the intake process and coordination of discharge care for clients that will be returning home.

Responsibilities

This position prefers prior home care, case management or discharge planning experience. Excellent planning, organization and presentation skills are critical.

  • Individual who is able to develop and build strong, lasting relationships with referral sources, clients and families.
  • Work in conjunction with facility staff to provide efficient and high quality discharge planning services for patients
  • Meet with prospective home health clients to discuss services
  • Patient advocacy
  • Obtain necessary authorizations and physician orders

Qualifications

    • Active VT RN or LPN nursing license preferred
    • Will consider other clinician with the right skills (COTA/PT/etc.)
    • Two (2) years clinical experience in a hospital or acute care setting is preferred
    • Knowledge of Medicare home health preferred
    • History of discharge planning experience and/or patient advocacy is preferred
    • Ability to travel to local facilities offices as needed
