BAYADA is seeking a Social Care Coordinator for our Center for Integrative Care.


Our Center for Integrative Care, which opened its doors in May of 2017, is dedicated to partnering with BAYADA offices across all specialty practices to recognize the holistic needs of our clients, and facilitate ongoing and additional services within the community. This new, revolutionary approach to client care looks beyond specialty care settings and responds to our clients' changing health care needs.


The Social Care Coordinator will work with the care coordinators and client advocates to drive integrated care for clients in the Philadelphia market.  The person who holds this dynamic role will work directly with clients who require assistance in applying for, or recertifying for, Medicaid and waiver programs, and coordinate other community resources or services. 



  • Support care planning for clients with complex and chronic conditions including performing and arranging for waiver services
  • Refer clients to community resources for appropriate agencies and organizations to enhance and supplement services for the client (i.e. Meals on Wheels, Transportation)
  • Document progress notes related to client contacts, advocacy, and care in accordance with BAYADA policies
  • Work with appropriate resources on evaluations for clients in need of discharge planning, transitions of care, or additional services
  • Facilitate authorizations and orders to coordinate services for clients
  • Develop and maintain relationships and knowledge of applicable community services
  • Facilitate patient access to recommended services



  • Four (4) year degree in Social Work, Psychology, or related field
  • Minimum of two (2) years’ work experience in human services, healthcare, mental health or related field
  • Experience with Medicaid strongly preferred
  • Experience with various PA Waiver Programs required
  • Ability to travel within Philadelphia market, utilizing own reliable transportation (apx 75% travel)
  • Excellent organizational and interpersonal skills
  • Ability to work independently
  • Excellent communications skills with clients, families and other caregivers
